Job Overview Greater Meridian Health Clinic, Inc. is seeking a dedicated and highly skilled Certified Family Nurse Practitioner to provide comprehensive primary care services across diverse patient populations. The ideal candidate will possess a strong clinical background, demonstrating expertise in patient assessment, diagnosis, and treatment planning within a collaborative healthcare environment. This role offers an exciting opportunity to deliver high-quality care in various settings, including outpatient clinics, and community health programs. The successful applicant will be committed to advancing patient health outcomes through evidence-based practice and compassionate care. Duties Conduct thorough physical examinations and patient assessments to establish accurate diagnoses. Develop and implement individualized treatment plans in collaboration with patients and healthcare teams. Prescribe medications and therapies following HIPAA regulations, DEA guidelines, and clinical protocols. Manage acute and chronic conditions across multiple specialties such as internal medicine, geriatrics, pediatrics, behavioral health, and emergency care. Utilize electronic health record (EHR) systems like eClinicalWorks for documentation and medical records management. Provide patient education on disease prevention, medication administration, nutrition, and lifestyle modifications. Collaborate with physicians, specialists, and other healthcare professionals to ensure coordinated patient care. Participate in clinical research initiatives and quality improvement projects to enhance healthcare delivery standards. Engage in telehealth consultations to extend access to care for remote or underserved populations. Qualifications Valid certification as a Family Nurse Practitioner from an accredited program with current state licensure. Demonstrated experience working with diverse populations including pediatric, adult, senior care, hospice & palliative medicine, and individuals with developmental disabilities. Proficiency in EMR/EHR systems such as eClinicalWorks; experience with medical documentation is essential. Strong knowledge of physiology, anatomy, pharmacology (including CPT coding), and diagnostic evaluation techniques. Experience in acute care settings such as ICU or Level I/II trauma centers is highly desirable. Excellent client communication skills with the ability to educate patients effectively across age groups including toddlers and neonatal patients. Knowledge of infection control practices and HIPAA compliance standards ensuring confidentiality of medical records.
