Full Job Description Salary:

$130,000

• $175,000 We are seeking a compassionate and bilingual Nurse Practitioner (NP) or Physician Assistant (PA) to join our client's team and provide high-quality healthcare to adult patients.

This role involves working primarily in an office setting, with occasional home visits to ensure continuity of care for our patients.

Key Responsibilities:

Conduct comprehensive health assessments, including patient histories, physical exams, and diagnostic evaluations. Develop and implement patient-centered care plans, providing treatment, education, and follow-up care. Diagnose and manage acute and chronic conditions for adult patients. Prescribe medications and therapies in accordance with state regulations and scope of practice. Perform occasional home visits to patients who require care outside the office setting, ensuring their medical needs are met in a timely and respectful manner. Collaborate with patients, families, and caregivers to promote continuity of care. Communicate effectively with patients in Spanish or Portuguese, ensuring they fully understand their care plans. Document all patient encounters accurately and in a timely manner in the electronic medical record (EMR) system. Provide clear communication and updates to collaborating physicians and the care team. Work closely with physicians, medical assistants, and other healthcare staff to provide coordinated, high-quality care. Educate patients and families on preventive care, chronic disease management, and treatment plans.

Requirements:

Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) in NJ. Fluency in Spanish or Portuguese required. At least 1-2 years of experience in primary care, family medicine, or internal medicine. Experience working with diverse populations and underserved communities. Strong clinical judgment and critical-thinking skills. Excellent interpersonal and communication abilities. Cultural competence and sensitivity to the needs of diverse populations.

• The annual base salary range is $130k to $175k.
