Job Description

Primary Care Nurse Practitioner or Physician (MD/DO) - New City & Suffern, NY Part-Time | 16-20 Hours Per Week | Assisted Living Communities Visiting Docs is seeking a compassionate and experienced Nurse Practitioner or Physician (MD/DO) to provide onsite primary care services at assisted living communities located in New City and Suffern, New York . This position is ideal for a provider seeking a flexible, weekday schedule while caring for older adults in a collaborative, community-based healthcare setting. Position Details Part-time opportunity Approximately 16-20 hours per week Travel between assisted living communities in New City and Suffern, NY Weekday daytime schedule No overnight shifts No weekend obligation for this position Flexible scheduling based on provider availability and practice needs Key Responsibilities Conduct initial, routine, and follow-up primary care visits Diagnose and manage both chronic and acute medical conditions Prescribe medications and develop individualized treatment plans Collaborate with facility staff, specialists, caregivers, and family members Complete timely documentation within the electronic health record Deliver high-quality, patient-focused medical care to the geriatric population Required Qualifications Active New York State Nurse Practitioner or Physician (MD/DO) license Current DEA registration Active enrollment with both Medicare and Medicaid Ability to practice independently Nurse Practitioners must satisfy New York State's 3,600-hour independent practice requirement Previous primary care experience Preferred Qualifications Experience caring for geriatric patients Assisted living, long-term care, or post-acute care experience Strong clinical judgment, communication, and organizational skills Please Note Applicants must already be enrolled as Medicare and Medicaid providers.
