3001578270-2 Apply for this job now Job Description Oak Street Health is seeking a Nurse Practitioner to provide high-quality, patient-centered primary care to adults on Medicare in underserved communities. In this role, you'll deliver comprehensive assessments, manage chronic and acute conditions, and collaborate closely with a multidisciplinary care team to improve patient outcomes. You will focus on preventive care, education, and care coordination, leveraging data and best practices to support value-based care. Responsibilities Provide comprehensive primary care to a panel of adult Medicare patients, including acute, chronic, and preventative care visits. Conduct detailed patient assessments, including histories and physical exams, and develop individualized care plans. Manage chronic diseases such as diabetes, hypertension, COPD, and heart failure, emphasizing prevention and care gap closure. Order, interpret, and act on diagnostic tests and laboratory results in a timely manner. Prescribe, adjust, and reconcile medications within scope of practice, ensuring safety and adherence. Document all patient encounters accurately and thoroughly in the Electronic Health Record (EHR). Collaborate closely with physicians, nurses, social workers, care managers, and other team members in a team-based care model. Participate in daily or regular care team huddles to review high-risk patients, coordinate follow-up, and optimize visit schedules. Provide patient and family education on disease management, medications, lifestyle changes, and preventative care. Engage in outreach and follow-up for high-risk patients, including post-discharge visits and chronic care management. Utilize telehealth when appropriate to enhance access, continuity, and patient convenience. Monitor and contribute to performance on quality, safety, and patient experience metrics. Participate in quality improvement initiatives, case conferences, and training sessions as needed. Adhere to all regulatory, compliance, and organizational policies and procedures.
