Responsibilities:

Complete annual or post-discharge home health risk assessments for Medicare Advantage members Conduct thorough evaluations of past and current medical history, conditions, and medication regimens Educate patients on health conditions and their medications during each assessment Identify care gaps and provide actionable recommendations to address medical needs Documentation is essential, but no treatment planning or medication prescribing required

Desired Qualifications:

Active Nurse Practitioner License in Ohio MSN degree earned from an accredited nursing school Current Board Certification from AANP or ANCC Valid and up-to-date CPR certification (BLS) A minimum of six months of recent Nurse Practitioner experience is preferred

Earn between $80 and $90 per evaluation, along with a mileage stipend and compensation for administrative time associated with case documentation and assessment preparation.
