Job Description

Deliver high-quality patient care in a rewarding per diem opportunity as a Nurse Practitioner specializing in home health assessments. This position offers flexibility and meaningful patient interactions by conducting thorough, patient-centered health risk evaluations for Medicare Advantage members throughout Shelby County. Enjoy the autonomy to create your schedule and work within your preferred locations, including Sidney, Fort Loramie, and Anna.
Schedule:
Flexible shifts available Monday through Sunday Design your own work schedule Set your weekly or bi-weekly hourly commitments
Key Responsibilities:
Perform annual home health risk assessments for Medicare Advantage members Gather and review comprehensive patient medical histories and medication regimens Educate patients on medical conditions and appropriate medication usage Identify care gaps and provide actionable recommendations for optimal health outcomes No requirement for treatment plan creation or prescribing of medications
Qualifications:
Active Nurse Practitioner License in the state of Ohio Up-to-date CPR certification (BLS) Board Certification through AANP or ANCC Minimum of 6 months current NP experience preferred MSN degree from an accredited nursing program Compensation for this opportunity is highly competitive, with $80-$90 per evaluation, plus a mileage stipend and additional compensation for administrative time.
