Job Description

Make an impact in home health care by leveraging your skills as a Nurse Practitioner. This per diem opportunity focuses on conducting home health risk assessments for Medicare Advantage plan members across Waverly and surrounding areas in Pike County, OH. Each assessment typically takes 45 minutes and may vary from annual health evaluations to post-discharge follow-ups. Your expertise ensures that patients receive quality healthcare insight and guidanceall without requiring you to develop treatment plans or prescribe medications.
Schedule:
Flexible shifts available Monday through Sunday Personalize your workdays by creating your own schedule Minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
Perform comprehensive home health risk assessments for Medicare Advantage members Review prior and current medical history, medical conditions, and medication use Offer education to patients on their health conditions and prescribed medications Identify gaps in care and provide actionable recommendations Complete all documentation with attention to accuracy and compliance
Qualifications:
Active Nurse Practitioner license in Ohio Current CPR certification (BLS required) Board Certification (AANP or ANCC) Minimum of 6 months' recent experience as an NP preferred Masters degree in Nursing (MSN) from an accredited institution
