Job Description

Our client in Ohio seeks a dedicated Family Practice Nurse Practitioner for a 16-day assignment. This role involves Monday, Tuesday, Wednesday, and Friday shifts from 9a-5p, focusing on comprehensive patient evaluations, follow-up care, and diligent charting. The practitioner will manage preventative health, transitional care evaluations, chronic care, acute sick visits, and routine follow-ups. Responsibilities and Duties Conduct patient evaluations Provide follow-up care Maintain accurate charting Perform preventative health assessments Manage transitional care evaluations Oversee chronic care management Address acute care/sick visits
