Job Description

Comhar Inc., a mission-driven nonprofit in Philadelphia, seeks a Psychiatric Mental Health Nurse Practitioner to provide compassionate, recovery-focused care for children, adults, and families in underserved communities. The PMHNP will conduct psychiatric evaluations, develop evidence-based treatment plans, and manage psychotropic medications in collaboration with an interdisciplinary team. This role offers rich clinical and community exposure, ongoing training, and a supportive, client-centered environment that values cultural humility, ethics, and continuous learning. Hybrid schedule options may be available. Responsibilities Conduct comprehensive psychiatric evaluations and diagnoses using evidence-based standards. Develop and manage individualized treatment and recovery plans for children, adolescents, and adults. Provide medication management, monitoring efficacy, safety, and side effects. Deliver crisis intervention and risk assessments, including suicide and safety assessments. Collaborate with interdisciplinary teams (therapists, social workers, case managers) to coordinate care. Document assessments, treatment plans, and progress in electronic health records (EHR) in a timely manner. Engage clients and families in psychoeducation and shared decision-making. Ensure culturally responsive and trauma-informed care aligned with organizational values. Participate in case conferences, supervision, and quality improvement initiatives. Maintain compliance with regulatory, ethical, and documentation standards.
