Nurse Practitioner / Physician Assistant (Community-Based Care) We are seeking an experienced Nurse Practitioner or Physician Assistant to provide comprehensive primary and preventive care to adult and geriatric patients in a community-based setting. This is a full-time, temporary assignment covering a maternity leave, offering a predictable weekday schedule and excellent work-life balance. Schedule Full-time, 40 hours per week Monday through Friday 8:00 AM to 5:00 PM No call, weekends, or holidays Approximate duration: 4 months Responsibilities Perform comprehensive patient assessments Manage acute and chronic medical conditions Develop and implement treatment plans Document encounters accurately and thoroughly in the EMR Collaborate with interdisciplinary healthcare teams Conduct advance care planning discussions Educate patients and caregivers on treatment plans and health management Ensure timely and compliant clinical documentation Requirements Active Nurse Practitioner or Physician Assistant license, or ability to obtain prior to start
NP:
Master's or Doctorate in Nursing from an accredited program
PA:
Graduate of an accredited Physician Assistant program National board certification as an NP or PA Current DEA license/prescriptive authority Reliable transportation and willingness to travel locally throughout the workday Ability to perform physical assessments and meet the physical demands of the role Ability to obtain a collaborative practice agreement if required by state law Preferred Qualifications At least 1 year of clinical experience Experience working with geriatric or medically complex populations Long-term care, post-acute, home-based, or community health experience Knowledge of chronic disease management and advanced illness care Strong documentation and EMR skills Location Providence, Rhode Island area Travel between approximately 5 facilities during the workday This opportunity is ideal for a provider who enjoys caring for complex patient populations while maintaining a consistent weekday schedule.
