PA-C/FNP-C

with Be Well Primary Care, you will provide comprehensive primary care services to our adult patient population, including: Evaluate, diagnose, and treat acute and chronic medical conditions Perform comprehensive patient examinations and routine follow-up visits Develop and manage individualized treatment plans Order, review, and interpret laboratory testing and diagnostic studies Prescribe medications within your scope of practice and prescriptive authority Manage chronic conditions such as diabetes, hypertension, hyperlipidemia, and other common primary care conditions Provide preventive care, health education, and lifestyle counseling Complete accurate and timely documentation in the electronic medical record (eClinicalWorks) Coordinate patient care with physicians, specialists, clinical staff, and other members of the healthcare team Provide appropriate referrals and follow-up care Maintain compliance with applicable federal and Texas regulatory requirements, clinical standards, and Be Well Primary Care policies and procedures Participate in quality improvement initiatives and other clinical responsibilities as needed What We're Looking For Current and unrestricted Texas PA or APRN license Current national board certification as a PA-C or FNP-C Current DEA registration or ability to obtain prior to beginning independent prescribing responsibilities Current BLS certification Prescriptive authority in accordance with Texas requirements Primary care experience strongly preferred Strong clinical judgment and decision-making skills Excellent communication and interpersonal skills Ability to work effectively both independently and as part of a collaborative healthcare team Strong organizational and time-management skills Commitment to providing exceptional, patient-centered care Comfortable working in a fast-paced outpatient primary care environment Ability to maintain accurate and timely patient documentation What We Offer At Be Well Primary Care, we want our providers to have the support they need to focus on what matters most — taking care of their patients.
