Medical Director/Clinic Director / Chief Nursing Officer (or designee) Position Summary The Evening Clinic Nurse Practitioner provides primary and urgent care services during extended clinic hours (approx. 2:00 PM
10:00 PM) to improve access to timely care and reduce non-emergent emergency department utilization.
This role focuses on same-day and after-hours care, management of acute and chronic conditions, and coordination of follow-up services to ensure continuity of care. The position is a key component of the organization's strategy to shift care from reactive emergency services to accessible, community-based primary care. Primary Responsibilities 1. Direct Patient Care
Provide comprehensive primary care services, including:
Acute illness evaluation and treatment
Chronic disease management
Preventive care services
Diagnose, treat, and manage a wide range of patient conditions
Prescribe medications and therapies in accordance with scope of practice
2. Same-Day and After-Hours Access
Deliver care for walk-in and same-day appointments
Provide an accessible alternative to emergency department use for non-emergent conditions
Ensure timely evaluation and treatment of patient needs 3. Chronic Disease Management
Manage patients with: o Diabetes o Hypertension o Cardiovascular conditions Coordinate care with:
Transitional Care RN
Population Health Coordinator
Registered Dietician 4. Care Coordination and Follow-Up
Ensure appropriate follow-up for patients seen during evening hours
Coordinate referrals to: o Home-based Nurse Practitioner/Physician Assistant (for high-risk patients) o Primary care providers o Specialty services as needed o Support continuity of care across the care team 5. Integration with Care Model
Collaborate with care coordination staff to:
Reduce avoidable ED utilization
Improve access to care
Support population health initiatives
Utilize patient data and care plans to guide treatment decisions 6. Documentation and Compliance
Document all patient encounters in the EHR accurately and timely
Ensure compliance with organizational policies and regulatory requirements
Support quality reporting and performance improvement initiatives Performance Expectations
Increase same-day and after-hours appointment availability by ≥20%
Contribute to a 10-15% reduction in non-emergent ED visits by Year 3
Maintain high patient satisfaction and quality of care
Ensure timely follow-up and continuity of care for patients seen Required Qualifications
Current Nurse Practitioner (NP)/Physician Assistant (PA) license in the State of Texas
Board certification as a Nurse Practitioner (NP), Adult-Gerontology (NP) and Physician Assistant (PA)
Prescriptive authority in accordance with Texas regulations
Minimum of 2-3 years clinical experience preferred Preferred Qualifications
Experience in: o Primary care or urgent care settings o Rural or underserved populations o Chronic disease management o Familiarity with EHR systems (e.g., CPSI/TruBridge) Skills and Competencies
Strong clinical decision-making and diagnostic skills
Ability to manage a wide range of patient conditions independently
Excellent communication and patient engagement skills
Ability to work efficiently in a fast-paced environment

Pay:
From $150,000.00 per year
