Job Summary We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our multidisciplinary healthcare team. In this vital role, you will provide comprehensive primary and specialty care across diverse patient populations, including pediatrics, geriatrics, and adults. Your expertise will support patient health through thorough assessments, diagnosis, treatment planning, and medication management. You will collaborate closely with physicians, specialists, and other healthcare professionals to deliver exceptional patient-centered care in a fast-paced environment. Duties Conduct detailed physical examinations, health assessments, and vital sign monitoring for patients of all ages. Develop and implement individualized treatment plans in collaboration with the healthcare team. Prescribe medications and therapies in accordance with state regulations and collaborative practice agreements. Manage acute and chronic conditions, including urgent care needs such as infections, injuries, and pain management. Perform procedures such as suturing, IV insertions, phlebotomy, and sterile technique-based interventions. Utilize electronic health record (EHR) systems like Epic or eClinicalWorks to document patient encounters accurately and efficiently. Provide patient education on health promotion, disease prevention, medication adherence, and lifestyle modifications. Participate in clinical research initiatives and quality improvement projects to enhance patient outcomes. Collaborate with specialists in areas such as gastroenterology, infectious disease care, behavioral health, geriatrics, hospice & palliative medicine, and occupational health to ensure comprehensive care delivery. Engage in telehealth consultations to extend access to care for remote or underserved populations. Skills Proven experience working with diverse patient populations including children, seniors, and individuals with disabilities or developmental challenges. Strong proficiency with EMR/EHR systems such as Epic, Cerner or Athenahealth for medical documentation and data management. Knowledge of advanced practice provider regulations including prescribing medications under collaborative practice agreements. Skilled in performing clinical procedures like suturing, catheterization, spinal taps, dermal fillers (nice-to-have), and tube feeding management. Familiarity with CPT coding and ICD coding (ICD-10/ICD-9) for accurate documentation and billing processes. Critical care experience including ICU or Level I/II trauma center exposure is highly desirable. Experience working within hospital medicine settings such as emergency rooms or inpatient units is advantageous. Ability to perform triage effectively in urgent care or emergency scenarios while maintaining excellent client communication skills. Competence in managing complex cases involving acute pain management, infectious diseases, dialysis support, ventilator management or post-operative care. Strong physiology knowledge combined with clinical assessment skills to support diagnostic evaluation across multiple specialties including cardiology (e.g., cardiac catheterization), ophthalmology or neuro-related procedures like spinal taps.
