Position Summary:
Revere Health is seeking a dedicated and compassionate Advanced Practice Practitioner (APP)—either a Nurse Practitioner (NP) or Physician Assistant (PA)—to join our dynamic Cardiology team. This role supports our mission to deliver high-quality, value-based care to patients with cardiovascular conditions.
Essential Job Functions:
Conduct comprehensive patient assessments, including history, physical exams, and diagnostic evaluations. Collaborate with cardiologists to develop and implement individualized treatment plans. Manage chronic cardiovascular conditions and provide follow-up care. Educate patients and families on disease prevention, treatment plans, and lifestyle modifications. Participate in quality improvement initiatives and interdisciplinary team meetings. Document patient encounters accurately and timely in the EMR system.
Qualifications:
Current licensure as a Nurse Practitioner or Physician Assistant in the state of Utah. Certification in cardiology or relevant clinical experience preferred. Strong clinical assessment and diagnostic skills. Excellent communication and teamwork abilities. Commitment to patient-centered, value-based care.

Monday - Friday 8 - 5
