Position Details Setting:
Inpatient consult service at Maryview Medical Center in Portsmouth, VA Potential for Outpatient clinic coverage at our Harbour View Palliative Care Clinic in Suffolk, VA Rare cross-coverage possible at Mary Immaculate Hospital in Newport News, VA Adult/Geriatric population only Average 6-8 patients per day
Schedule:
Full-time, Monday-Friday, 7:00 AM-5:00 PM Four 10-hour shifts with a rotating day off No call What You'll Do Provide expert palliative care services to patients, focusing on symptom management, emotional support, and improving quality of life. Work closely with inter disciplinary team to deliver holistic and patient-centered care Provide comprehensive palliative care consultations to patients with serious, chronic, or terminal illnesses. Manage symptoms such as pain, nausea, fatigue, and breathlessness through pharmacologic and non-pharmacologic interventions Conduct meaningful goals-of-care conversations, advance care planning, and end-of-life discussions Facilitate patient and family discussions around goals of care, advance directives, and treatment preferences. Collaborate with physicians, social workers, chaplains, and other team members to create individualized care plans. Assist in transitioning patients to hospice care when appropriate, ensuring continuity of care. Use robust tools including EPIC , MyChart, and DAX CoPilot/AI documentation support Contribute to a warm, patient-centered environment where dignity, comfort, and communication are key What We're Looking For
MUST HAVE

Required:
3-5 years of Nurse Practitioner practice experience in the palliative care specialty, critical care and/or hospice care. Applicants who do not meet minimum eligibility requirements will not be considered.
Minimum:
Master of Science in Nursing, current Virginia Practice Licensure (or ability to obtain) and Board Certification as FNP, ACNP or
AGNP BLS
