Position Description We are looking for a dedicated Nurse Practitioner to serve as the primary care provider for our skilled nursing facility in St. Albans, VT. This is a full-time, in-person position, Monday through Friday during daytime hours, with no nights, no weekends, and no on-call. You will manage a panel of long-term care and short-stay rehab residents, handling chronic disease management, acute changes in condition, and coordination with the facility's nursing and therapy teams. Nurse Practitioner:

Active and unrestricted Vermont Nurse Practitioner license in good standing Active DEA registration Board certification as an FNP, AGNP, or AGPCNP Experience in skilled nursing, long-term care, or geriatrics preferred Responsibilities of the

Nurse Practitioner:
