Job Description

Overview Join our compassionate healthcare team as a Home Health Aide / Certified Nursing Assistant (CNA) and make a meaningful difference in the lives of patients requiring personalized home care. In this energetic role, you will provide essential support to individuals across various stages of health and wellness, promoting independence and comfort within their own homes. Your dedication will help foster a safe, respectful, and nurturing environment for clients with diverse needs, including geriatric patients, those with developmental disabilities, and individuals receiving hospice or memory care. This position offers an opportunity to utilize your caregiving skills in a dynamic setting that values your commitment to patient well-being. Duties Deliver high-quality personal care assistance, including helping patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts. Monitor patient health by observing vital signs, administering medications as prescribed, and performing catheter care when necessary, all while adhering to home healthcare regulations and maintaining HIPAA compliance to ensure patient confidentiality. Assist with medical tasks such as wound care, basic life support (BLS), CPR certification, and first aid to ensure safety during emergencies or routine care. Support patients' nutritional needs through meal preparation and light cleaning duties to promote a healthy living environment. Document medical records accurately using Electronic Health Record (EHR) systems or Electronic Medical Records (EMR systems), ensuring clear communication among healthcare providers. Provide companionship and emotional support for clients dealing with dementia, Alzheimer's disease, or end-of-life care needs in a sensitive and respectful manner. Facilitate effective communication with clients and their families to understand their preferences and update care plans accordingly. Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare settings is highly preferred. Demonstrated experience working with individuals with developmental disabilities, dementia, or geriatric patients enhances your ability to deliver specialized care. Familiarity with home health care assistance regulations, medical terminology, medication administration protocols, and patient transfer methods is essential. Proven ability to perform heavy lifting safely and operate equipment such as Hoyer lifts or patient transfer devices. Experience working within hospice or memory care environments demonstrates your capacity for compassionate end-of-life support. Strong communication skills combined with experience documenting medical information accurately in medical records or EHR/EMR systems are vital for success in this role. Certification in CPR and First Aid is required; additional certifications such as Basic Life Support (BLS) are advantageous.
