Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene Monitor patient health by taking vital signs such as blood pressure, pulse, and temperature; observe and report any changes or concerns Support medication administration according to care plans and ensure compliance with HIPAA regulations for patient privacy Prepare nutritious meals tailored to dietary needs and assist with feeding when necessary Facilitate patient transfer methods using equipment like Hoyer lifts and manual handling techniques to ensure safety during mobility tasks Provide companionship and emotional support, engaging clients in meaningful activities to promote mental well-being Maintain cleanliness of the client's home environment through light housekeeping and sanitation tasks Assist with transportation for medical appointments, errands, or community outings while ensuring client safety and comfort Follow detailed care plans designed for each individual's unique needs, including dementia, Alzheimer's, or neurodevelopmental disorders Document care activities accurately and communicate effectively with healthcare teams regarding patient status or concerns Experience Prior experience working in home care, assisted living facilities, nursing homes, or group homes is highly preferred Demonstrated knowledge of patient observation techniques and monitoring vital signs Experience assisting with complex medical needs such as medication administration, hospice care, or Alzheimer's/dementia care support. Familiarity with patient transfer methods including Hoyer lifts and safe manual handling practices Background working with individuals with disabilities, developmental or intellectual disabilities, autism spectrum disorder or neurodevelopmental disorders is advantageous Strong understanding of client's rights and confidentiality standards under HIPAA regulations Hands-on experience with care planning, patient personal care assistance, and supporting long-term care needs About
