In this role, you will provide respectful, attentive care that promotes safety, independence, dignity, and overall well-being. You will assist members with daily living activities, personal care, mobility, health-related needs, and community participation while supporting their individual goals and preferences. This paid position offers an opportunity to use your CNA experience in a supportive environment focused on quality care and compassionate service. Responsibilities Provide hands-on, person-centered care while maintaining each member's dignity, privacy, and independence Assist members with activities of daily living, including bathing, grooming, dressing, toileting, eating, and personal hygiene Support mobility, transfers, positioning, and safe use of mobility equipment as needed Monitor and report changes in physical, behavioral, or emotional well-being to appropriate supervisors and healthcare professionals Assist with meal preparation, feeding, hydration, and other nutritional needs according to individual care plans Follow individualized service plans, healthcare instructions, safety protocols, and agency policies Provide support with medication reminders or medication-related tasks as permitted by training, certification, and applicable regulations Maintain a clean, safe, and comfortable environment for members Support individuals with disabilities in developing independence and participating in meaningful daily and community activities Provide companionship, encouragement, and emotional support while respecting individual preferences and choices Document services provided, observations, incidents, and changes in condition accurately and promptly Respond appropriately to emergencies and follow established procedures, including CPR and first aid when required Communicate professionally with families, supervisors, healthcare providers, and other members of the care team Skills & Qualifications Current Certified Nursing Assistant (CNA) certification preferred/required as applicable to the position Experience providing direct care to individuals with disabilities, older adults, or individuals with complex support needs is highly valued Knowledge of personal care, activities of daily living, infection control, and basic healthcare practices Ability to safely assist with transfers, mobility, positioning, and personal care CPR and First Aid certification or willingness to obtain certification Strong observation and communication skills Reliable, patient, compassionate, and respectful approach to caregiving Ability to follow individualized care plans and maintain accurate documentation Ability to work independently while also collaborating effectively with a care team Comfortable supporting individuals with physical, developmental, behavioral, or communication-related needs Ability to maintain confidentiality and professional boundaries Dependable transportation and willingness to meet members in their home or community, as required Commitment to promoting dignity, choice, independence, and quality of life for every member
