Responsibilities Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts. Monitor and document patient observations, including vital signs, medical conditions, and behavioral changes, ensuring accurate medical documentation and adherence to healthcare regulations. Support patients with medication administration under supervision, catheter care, and other medical procedures while maintaining strict confidentiality in compliance with HIPAA guidelines. Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care strategies and end-of-life support. Use Electronic Health Record (EHR) systems to update patient records promptly and accurately; communicate effectively with healthcare teams regarding patient status. Assist with meal preparation, feeding, laundry, cleaning, and other home health care assistance tasks to promote a safe and comfortable environment. Follow all safety protocols during patient transfers or heavy lifting activities; utilize proper patient transfer methods to prevent injury. Engage in ongoing training such as first aid, CPR, basic life support (BLS), and home health care regulations to ensure high-quality patient care. Qualifications APD training courses (Will allow a few days to attain if needed) Valid certification as a Nurse Aide or Personal Support Worker with current state licensing or registration. Proven experience working in assisted living facilities, nursing homes, or home health environments. Strong knowledge of medical terminology, medication administration procedures, catheter care, and vital signs measurement. Experience working with individuals with disabilities or developmental disabilities. Ability to communicate effectively with patients, families, and healthcare teams; compassionate caregiving attitude essential. Competency in using EMR/EHR systems for documentation; understanding of HIPAA regulations to maintain patient confidentiality. Physical stamina for activities such as heavy lifting and patient transfers; ability to work flexible hours including live-in employment if required.
