Job Overview We are seeking a compassionate and dedicated Home Caregiver - HHA/CNA to join our team and provide exceptional in-home care to clients across diverse needs. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs, managing medication administration, and reporting changes promptly Provide specialized care such as Alzheimer's care, dementia support, hospice assistance, and end-of-life care with empathy and professionalism Operate Electronic Health Record (EHR) systems to document medical records, care notes, and patient interactions accurately Support safe patient transfers using proper methods including Hoyer lifts and patient transfer techniques Maintain patient confidentiality in accordance with HIPAA regulations while handling sensitive medical information Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment Requirements Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) with current licensing where applicable Proven experience working with individuals with disabilities, geriatrics, or in assisted living environments Strong knowledge of home healthcare regulations, medical terminology, and patient care standards Ability to perform physical tasks including heavy lifting and assisting with mobility aids safely Experience working with EMR/EHR systems for documentation and communication purposes Certifications in CPR and First Aid; additional training in hospice or dementia care is a plus Excellent communication skills to effectively interact with clients, families, and healthcare teams Reliable transportation and flexibility to work various shifts including live-in arrangements if needed Join us in delivering compassionate home health care that truly makes a difference!
