Looking for 1st. Shift 6a-2p. FT and 3rd. Shift 10p-6a.

PT PURPOSE OF YOUR JOB DESCRIPTION

To work closely with the Charge Nurse and other personnel of Heritage Square in rendering professional nursing care to all residents.

MAJOR DUTIES AND RESPONSIBILITIES

1. Follow all established safety procedures and precautions, according to current OSHA and Insurance regulations. 2. Completes walking report with 10-6 CNA's. 3. Receives floor assignment from charge nurse. Helps other aides with residents when needed. Answer call lights. 4. Obtain ice and water from SC dining room and pass fresh water to residents in the morning. Pass ice water during shift. 5. Answer call lights and alarms from patient rooms and other care areas to determine any patient need. 6. Bathes, dresses, and undresses residents and assists with their personal hygiene. Cleans and disinfects tub and shower chair after each use.

Note:

Each resident is to have two baths or showers per week. Apply hearing aides, eyeglasses, and dentures if indicated. 7. Serves and sets up food trays for residents on the skilled unit. Assists the dietary person in serving the residents. Feed residents requiring help. Provides fresh drinking water to all residents. Provide meal nourishment as directed. 8. Encourage and assist residents with any activity, recreational or occupational. 9. Transport residents to treatment areas using wheelchairs or assisting them to walk. 10. Take and record vital signs (weekly and monthly) in a timely manner and record these in the appropriate place. Maintain food and fluid consumption records, 1&0, daily charting (B&B, turning schedule, etc.), 7 day and 30 day tracking, care plan charting and any other charting as directed by the Charge Nurse or the Director of Nursing. 11. Report all unusual or change in conditions to the Charge Nurse. 12. Change bed linen as needed. 13. Collect and bag all soiled linens and/or personal clothes. 14. Pass clean linen. 15. Make sure that dining room, utility rooms, and nurse's station are straightened up before going off duty. 16. Maintain current CPR certification. 17. CNA is directly responsible to the Charge Nurse. 18. Complete kardex plan of care for all new residents and maintain. 19. Be aware of fire exits, fire alarms, and fire procedures. 20. Attend at least 12 hour of inservice training every year. 21. Clean wheelchairs and walkers as scheduled. 22. Go to doctor's appointments with residents. 23. Each CNA is responsible for checking the working schedule frequently, as changes will and do occur on a moment's notice due to call-offs, staff resigning, LOA, etc. All schedule requests must be given to the Director of Nursing in writing before the schedule is posted or the request will not be granted. Once the schedule is posted, it is the responsibility of the staff to find their own replacement and that can cause nobody to be in Overtime. If you are requesting your weekend to work off, you must find your own replacement. 24. Employees are allowed (2) IO-minute breaks and (1) 30-minute lunch break. All breaks are to be assigned by Charge Nurse at the beginning of the shift. 25. Is responsible for cleaning schedule and equipment as assigned by Charge Nurse.
