Duties Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility Monitor and observe patient conditions, including vital signs and overall health status Support medication administration and manage medical documentation accurately using Electronic Health Record (EHR) systems Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments Assist with catheter care, wound care, and other complex medical procedures following home healthcare regulations Transfer patients safely using patient transfer methods like Hoyer lifts or gait belts while maintaining proper body mechanics Communicate effectively with clients and family members to ensure personalized care plans are followed and needs are met Skills Strong knowledge of medical terminology, HIPAA regulations, and patient confidentiality standards Experience working with geriatric patients, individuals with developmental disabilities or disabilities requiring specialized care such as hospice or end-of-life support Proficiency in using EMR/EHR systems for documentation and reporting Ability to perform patient personal care assistance including feeding, toileting, and hygiene support Knowledge of home health care assistance techniques including cooking, cleaning, laundry, and light housekeeping tasks Familiarity with patient transfer methods and equipment such as Hoyer lifts or transfer belts Certification in First Aid and Basic Life Support (BLS), including CPR training; experience in heavy lifting is preferred for safe patient handling Join us to be part of a dedicated team committed to delivering compassionate home health care that truly makes a difference. Pay:
