Overview Join our dedicated home care team as a Certified Nursing Assistant (CNA) and make a meaningful difference in the lives of our clients. You will assist with daily living activities, personal care, mobility, and safety while providing compassionate, respectful, and dependable care in the home. Responsibilities Provide direct client care by assisting with activities of daily living, including bathing, dressing, grooming, toileting, mobility, and transfers. Safely assist with transfers and repositioning using proper body mechanics and equipment such as Hoyer lifts and gait belts when required. Monitor and accurately record vital signs, including blood pressure, pulse, temperature, and respirations. Provide medication reminders and assist with medication-related tasks within the CNA scope of practice and agency policy. Follow infection control, safety, and fall-prevention procedures at all times. Communicate effectively with clients, families, supervisors, and members of the care team regarding changes in condition or care needs. Complete accurate and timely documentation of care provided using the agency's electronic documentation system while maintaining client confidentiality and HIPAA compliance. Requirements Valid, current certification as a Certified Nursing Assistant (CNA) in the state where services are provided. Must be able to safely lift, transfer, push, or pull up to 50 pounds as required when assisting clients. Current CPR/BLS certification required. Experience providing care to older adults, individuals with disabilities, and clients with conditions such as dementia, Alzheimer's disease, or other memory-related needs is preferred. Ability to assist with activities of daily living, including bathing, dressing, grooming, toileting, mobility, transfers, meal preparation, and other personal care needs. Experience using transfer equipment, including Hoyer lifts, gait belts, and other assistive devices , is preferred. Ability to accurately document care provided, observations, and required clinical or caregiver notes using the agency's electronic documentation system or mobile application. Knowledge of infection control, client safety, fall prevention, and proper body mechanics. Strong communication and interpersonal skills with the ability to interact professionally and compassionately with clients, families, supervisors, and other members of the care team. Must demonstrate reliability, professionalism, respect for client privacy, and the ability to follow the client's individualized plan of care. Prior home care, long-term care, assisted living, rehabilitation, or similar caregiving experience is preferred. Additional certifications, such as Medication Technician/Medication Administration training, may be considered a plus when applicable to the position.
