In this vital role, you will provide personalized assistance to individuals in their homes or care facilities, ensuring their comfort, safety, and dignity. This position offers an energizing opportunity to work closely with diverse patient populations, including seniors, individuals with disabilities, and those receiving hospice or memory care services. Your expertise will support their well-being while fostering a trusting and respectful environment. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor and document patient conditions by observing vital signs such as blood pressure, pulse, respiration, and temperature using medical terminology accurately Support medication administration following home healthcare regulations and proper medical documentation procedures Perform patient transfers using safe methods like Hoyer lifts or patient transfer techniques to ensure safety for both patients and caregivers Maintain patient confidentiality in compliance with HIPAA regulations while managing medical records and electronic health record (EHR) systems Provide specialized care such as catheter care, dementia or Alzheimer's care, end-of-life support, and memory care tailored to individual needs Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment for clients Communicate effectively with patients and family members to address concerns and provide emotional support during home health visits or hospice care Requirements Valid certification as a Certified Nursing Assistant (CNA) or Home Health Aide (HHA) from an accredited program Proven experience working with geriatric patients, individuals with developmental disabilities, or in assisted living environments Knowledge of medical terminology, medication administration protocols, and home health care regulations Ability to perform patient personal care assistance safely and compassionately while maintaining patient dignity CPR and First Aid certification; Basic Life Support (BLS) training preferred Physical ability to lift heavy objects when necessary and perform patient transfer methods safely Strong communication skills for client interaction and team collaboration while maintaining HIPAA compliance Join us in delivering exceptional home health care that enriches lives through compassionate service. Pay:
$18.00 - $20.00 per hour Expected hours: 12.0 - 36.0 per week
