Job Description

Northport Highlands Assisted Living and Memory Care Direct Care Worker and Med Passer Job Description Job Summary:
Provides direct care to residents. Assist /provide residents with activities of daily living, laundry, and housekeeping services. Works with Administrator/DON to meet and maintain licensing regulations.
Physical Job Requirements:
Must be able to stand/walk for long periods of time; bend/stoop; lift/carry/reach over 50 lbs., and assist residents with ADL's and transfers.
Typical Duties:
Assist the Administrator/DON in providing a clean and safe environment for residents. See that the rules established by the Administrator/DON are followed Assisting with maintenance of resident and facility records Dispense medications to residents as prescribed by a physician and according to state regulations Assist and participate in fire drills and other required drills, activities and state regulations Implement and document individual resident programs as designed by the Administrator/DON including, but not limited to the following: Resident and facility laundry Assist with weight and vitals Housekeeping throughout facility Maintaining resident activities of daily living Showering and bathing residents Maintaining resident hygiene and nutrition Attend staff meetings/in services/policy and procedure reviews Documentation of hours, education, training required by state or local regulations Responsible for maintaining supplies for the residents and facility, such as paper towels, hand soap, toilet paper, etc. Keep linen closets neat and clean with items folded Communicate resident needs to the Administrator/DON, i.e. depends, clothes, personal items, toothpaste…. Inform Administrator/DON of problems, concerns, family contacts, hospital admissions, medication changes or changes in resident condition, physically or mentally Other job duties as assigned by Admin/DON General Responsibilities for all staff: Responsible for the general well being and safety of the residents Treat residents and staff with dignity and respect Participation in all required surveys, licensing requirements, fire drills, policy and procedure reviews, etc. Maintenance, upkeep and cleaning of facility Teamwork and participation Communication with Administrator/DON Greeting residents, families, visitors, peers, other agency personnel with respect and friendliness Answering phones: facility name , this is, state your name . Timely notification of absences, illness, etc. and completing appropriate forms as per policy/procedure Reporting any incidents to the Administrator/DON and documentation on appropriate forms Knowledge of and participation in licensing requirements Continuing education as dictated by job function Complete confidentiality as per policy in regard to resident information, staff and facility
Pay:
$21.00 - $22.00 per hour
