Job Summary Join our compassionate team as a Direct Caregiver (CNA / HHA), where you will play a vital role in delivering personalized, high-quality care to individuals across various settings. This job posting is specifically for a client of ours in Jordan, MN with the flexibility to add hours with other clients as well. The right person for this role will be near and willing to work in Jordan, MN and have experience working with aging populations, mobility challenges, and memory care. Additionally, the best fit for this particular client will be someone who is energetic, up beat, and takes initiative to find ways to help. Duties Provide patient personal care assistance, including bathing, dressing, grooming, and mobility support using patient transfer methods Assist with Activities of Daily Living (ADLs) such as eating, toileting, and medication reminders while maintaining patient dignity and confidentiality in accordance with HIPAA regulations Monitor client's day-to-day wellbeing; document records accurately using Electronic Health Record (EHR) systems or EMR systems Support clients with specialized needs such as Alzheimer's care, dementia care, memory care, or end-of-life care by following detailed care plans with the support of our RN team. Facilitate safe patient transfers and mobility using proper techniques to prevent injury for both clients and caregivers Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry and meal preparation in compliance with home health care regulations Experience Prior experience working with seniors or geriatric patients in a home setting, assisted living facilities, Skilled Nursing Facilities, or Memory Care Units strongly preferred Demonstrated caregiving experience in home healthcare settings or hospice environments is a plus Familiarity with medical terminology and medical documentation practices is a plus Experience working with individuals with developmental disabilities or disabilities requiring specialized support is a plus Knowledge of home & community care standards, HIPAA compliance, and patient confidentiality requirements Hands-on experience operating EMR/EHR systems for accurate record keeping and communication within healthcare teams Valid certification as a CNA (Certified Nursing Assistant) or HHA (Home Health Aide), along with current CPR/First Aid certification is preferred - we will train you on Touching Hearts at Home's standards.
