Position:

Home Health Certified Nursing Assistant (CNA)

Job Duties:

Provide one-on-one personal care to patients in their homes following a patient-specific care plan developed by the RN Case Manager. Duties include bathing and personal hygiene, ambulation and transfer assistance, vital signs monitoring, and other activities of daily living (ADL). Collaborate closely with patients, families, caregivers, and other members of the interdisciplinary team. Complete point-of-care documentation accurately and timely. Meet the agency's productivity expectations.

Qualifications:

Current Oregon CNA license. Minimum of 1 year of full-time direct patient care experience. Prior experience with Electronic Medical Records (EMR) is preferred. Ability to maintain composure in stressful and challenging situations. Current and valid Oregon driver's license and car insurance. CPR certification. Reliable vehicle. Ability to function effectively as a member of the interdisciplinary team, communicating with the team at least weekly.
