Provide quality resident care within the CNA scope of practice and conduct assigned procedures under the direction of the Charge Nurse/Licensed Nurse Supervisor. Administrative Record all entries on flow sheets, notes, charts, etc., in an informative and descriptive manner
Report all changes in resident's condition to the Charge Nurse/Nurse Supervisor immediately Nursing Care Provide ADL assistance per care plan (from minimal assist to total care), including bathroom assistance, hygiene, grooming and meal prep/transport
Take and document vital signs, weights, intake/output and meal consumption | Report significant changes (e.g., weight or appetite)
Maintain resident comfort and safety (fresh water within reach, call light accessible)
Support exams/tests/appointments as assigned and observe resident response to interventions/treatments
Provide trauma-informed, person-centered care, including residents with dementia, behavioral health or psychosocial needs Care Plan and Assessment Participate in person-centered care planning and review care plans daily for changes
Communicate observations and concerns to nursing leadership per facility policy Admissions | Transfers | Discharges Assist nurses with admission, transfer and discharges | Greet and orient new residents
Introduce residents to roommates/staff and help inventory/label personal belongings Staff Development Attend required orientation, in-services and ongoing education | Maintain current CNA certification
Complete assigned competencies (e.g., resident rights, abuse prevention, dementia/behavioral care) Safety | Sanitation Follow infection prevention, PPE and safety procedures | Keep call light within reach
Clean/disinfect resident care equipment and shared devices | Conduct routine care-area housekeeping
Report hazards, safety concerns, equipment issues and communicable disease concerns per policy
Store tools/supplies appropriately and maintain required labels/SDS information Equipment | Supplies Report defective equipment promptly | Follow lockout/tagout procedures CNA Qualifications Shift and Hours Rotational Day & Night Shift | 6:00 AM to 6:00 PM and 6:00 PM to 6:00 AM Education Preferred | High School Diploma or Equivalent Certification Required | Texas Licensed Certified Nursing Aide (CNA) Experience | Skills | Knowledge Required Ability to provide age-appropriate care and follow standards of practice
Strong communication, teamwork and professionalism with residents, families, and staff
Comfort working with challenging behaviors | Uses patience, tact and empathy Preferred 1+ year experience in a skilled nursing facility Physical | Sensory Requirements Able to push, pull, move and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move and/or carry such weight a minimum distance of 50 feet
