Assist patients with bathing, dressing, grooming, toileting, and other personal-care needs.

Help patients safely walk, transfer, turn, and reposition.

Take and record vital signs, including temperature, pulse, respiration, and blood pressure.

Observe patients and report physical, emotional, or behavioral changes to the supervising nurse.

Assist with meals and feeding when needed.

Provide companionship and emotional support.

Keep the patients area clean, comfortable, and safe.

Follow the patients individualized care plan.

Document services provided accurately and on time.

Follow infection-control, privacy, and workplace-safety procedures.

Use medical equipment properly when trained and authorized.

Communicate professionally with patients, families, nurses, and other healthcare staff.
