Job Description

Touchstone Communities is seeking a compassionate Certified Nurse Aide to support elders in our long-term care community. As part of our family-oriented team, you will assist residents with daily living activities, including bathing, dressing, grooming, mobility, toileting, and meals. You'll measure and record vital signs, observe and promptly report changes, and help maintain a clean, safe, and dignified environment. Working closely with LVNs, nurses, and other caregivers, you'll contribute to tailored care plans and provide emotional support to residents and their families. CNA certification and commitment to teamwork and compassionate care are required. Responsibilities Provide hands-on assistance with bathing, grooming, dressing, toileting, and feeding residents Measure and record vital signs and report changes to nursing staff Support safe transfers, mobility, and positioning using proper techniques and equipment Maintain a clean, safe, and comfortable environment for residents Document care provided and observations in accordance with facility procedures Practice infection control and follow all safety and privacy regulations Partner with LVNs, RNs, and care team members to deliver coordinated, person-centered care Offer respectful, compassionate communication and emotional support to residents and families Required Skills Vital signs monitoring Assistance with activities of daily living (ADLs) Safe patient transfer and mobility techniques Infection control practices Basic medical terminology Electronic health record (EHR) documentation Observation and reporting of patient condition changes Pressure injury and fall prevention support Resident rights and confidentiality (HIPAA) Team-based care in long-term care setting
