Job Description

Job Summary We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) / Caregiver to join our team in providing exceptional patient-centered care. In this vital role, you will assist individuals with their daily needs, promote independence, and ensure a safe, comfortable environment. Your energetic approach and attention to detail will help improve the quality of life for our clients across various care settings, including home health, assisted living, and long-term care facilities. Duties Provide personal care assistance including bathing, dressing, grooming, and mobility support for patients with diverse needs. Assist with Activities of Daily Living (ADLs) such as toileting, feeding, and medication reminders to promote independence. Monitor patient health by observing vital signs like blood pressure, pulse, respiration, and temperature; report changes promptly. Support patients with mobility using proper patient transfer methods and equipment such as Hoyer lifts or gait belts. Maintain accurate medical documentation using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems; ensure compliance with HIPAA regulations to protect patient confidentiality. Administer medications as directed by healthcare professionals and assist with specialized care such as catheter care or dementia-related needs. Engage in patient communication to provide emotional support, explain procedures clearly, and foster trust and comfort. Requirements Valid CNA certification or equivalent caregiving credentials; previous assisted living or nursing home experience preferred. Demonstrated experience working with individuals with developmental disabilities, dementia, Alzheimer's disease, or in hospice/end-of-life care. Knowledge of medical terminology, medication administration protocols, and home healthcare regulations. Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts. Certification in First Aid and Basic Life Support (BLS), including CPR training; ongoing training preferred. Experience working with geriatric patients and familiarity with home & community care environments. Strong communication skills for effective client interaction and team collaboration; attention to detail for accurate medical records management. Ability to work independently or as part of a team in fast-paced settings; flexibility for live-in employment or varied shifts is a plus.
