Veterans Preference Case Manager/ Independent Living Coach Job Responsibilities :

The Case Manager in the role of Independent Living Coach assists the participants to meet program and operational objectives, Determine services and housing eligibility needs based on Treatment/Life Plan implementation. Organizational skills and ability to develop Treatment/Life Plans that are easily followed, measurable and attainable. Ability to work independently with minimum supervision.

Requirements:

Must have Bachelor's Degree in Human Services. 1+ years' experience working with adult population. Employer conducts level II Background Checks.
