Compensation:

$38-42/hr Position Summary Comfort Keepers of Lexington is seeking a compassionate and experienced Registered Nurse (RN) to serve as a Part-Time Care Manager. This position is responsible for conducting nursing assessments, developing individualized care plans, performing supervisory visits, and ensuring exceptional care for seniors and veterans receiving non-medical home care services. The RN Care Manager works closely with clients, families, caregivers, physicians, hospitals, and Veterans Affairs (VA) care coordinators to promote independence, safety, and quality of life while helping clients age comfortably in their own homes. Essential Responsibilities Conduct initial in-home nursing assessments for prospective clients. Develop, implement, and update individualized care plans based on client needs. Perform supervisory visits and reassessments in accordance with Kentucky regulations and Comfort Keepers policies. Monitor changes in clients' physical, cognitive, and emotional status. Collaborate with physicians, hospitals, rehabilitation facilities, hospice agencies, and other healthcare professionals. Coordinate care for veterans receiving services through the VA Community Care Network and other veteran programs. Provide caregiver orientation, clinical instruction, and ongoing coaching for client-specific care needs. Participate in caregiver competency evaluations and skills development. Maintain timely, accurate documentation of assessments, care plans, supervisory visits, and client communications. Assist with quality assurance initiatives and client satisfaction follow-up. Respond to client and family concerns and recommend appropriate solutions. Serve as a clinical resource for office staff and caregivers. Qualifications Current, unrestricted Registered Nurse (RN) license in the Commonwealth of Kentucky. Minimum of two (2) years of nursing experience. Experience in home health, geriatrics, hospice, case management, or community nursing preferred. Strong assessment, documentation, and care planning skills. Excellent communication and relationship-building abilities. Ability to work independently and manage a flexible schedule. Proficiency with electronic medical records and Microsoft Office applications. Valid Kentucky driver's license, reliable transportation, and proof of auto insurance. Preferred Qualifications Previous home care or private duty experience. Experience working with seniors and veterans. Knowledge of VA Community Care or veteran healthcare programs. Case management or care coordination experience. Supervisory experience in healthcare. Physical Requirements Frequent local travel throughout Fayette County and surrounding communities. Ability to safely enter clients' homes and conduct assessments. Ability to lift or carry up to 25 pounds occasionally. Schedule Part-time (15-25 hours per week) Primarily weekday business hours with flexibility to meet client needs.
