Job Summary We are seeking a dynamic and compassionate TCM Care Manager to lead patient-centered care coordination within a hospital setting. In this vital role, you will oversee comprehensive case management activities, ensuring seamless transitions across inpatient, outpatient, and post-acute care environments. Your expertise will facilitate optimal patient outcomes by coordinating services, managing utilization, and supporting discharge planning. Responsibilities Conduct thorough patient assessments, including medical records review, physiological evaluation, and psychosocial factors to develop personalized care plans. Collaborate with multidisciplinary teams—including physicians, social workers, behavioral health specialists, and addiction counselors—to coordinate inpatient and outpatient services. Manage utilization review processes, ensuring appropriate level of care and adherence to managed care protocols. Facilitate discharge planning by coordinating post-acute care services, outpatient clinics, and community resources to support smooth patient transitions. Perform diagnostic evaluations and case management activities for patients in
ICU, PICU
(Pediatric Intensive Care Unit), trauma centers (Level I & II), and other acute care settings. Maintain compliance with HIPAA regulations by managing sensitive medical records and safeguarding patient information throughout all processes. Lead intake procedures for new patients, including comprehensive medical terminology application and patient service documentation. Support behavioral health initiatives through mental health counseling and coordination of psychiatric services as needed. Utilize strong knowledge of hospital medicine, emergency medicine protocols, physiology concepts, and diagnostic tools to inform clinical decision-making. Skills Extensive hospital experience with a focus on acute care environments such as ICU, PICU, trauma centers (Level I & II), and emergency medicine settings. Proven expertise in managed care principles, utilization management, discharge planning, and post-acute care coordination. Strong understanding of medical terminology, medical records management, intake procedures, and diagnostic evaluation techniques. Experience in social work or behavioral health counseling with an emphasis on mental health support for diverse patient populations. Knowledge of inpatient and outpatient clinic operations along with familiarity with hospital medicine workflows. Ability to assess complex patient needs through detailed patient assessment skills rooted in physiology knowledge. Excellent communication skills for collaborating with multidisciplinary teams and engaging patients effectively. Familiarity with HIPAA compliance standards to ensure confidentiality of protected health information (PHI). The NC Medicaid TCM Provider Manual specifies at least two years of experience providing these services to the population being served for many care manager roles post graduation Completion of required North Carolina Tailored Care Management training before managing members independently, along with ongoing annual refresher training.

$38,000.00 - $45,000.00 per year
