THE POSITION

The Lebanon County Area Agency on Aging is seeking an Aging Care Manager 2 who is eager to coordinate programs that enrich the lives of older adults. This position allows you to support vulnerable adults while expanding your skills in protective service work. DESCRIPTION OF WORK

This role focuses on providing protective services for older adults and some younger adults who require assistance. It involves receiving reports, conducting investigations, and coordinating care plans that support safety and well-being. As an Aging Care Manager 2, you will perform the following duties:

Protective Service Reports:

Receive and investigate reports of need for protective services for older adults

Care Plan Development:

Assess needs and coordinate service plans that support safety and reduce risks

Regulatory Compliance:

Follow state regulations and required procedures after obtaining certification

Interagency Coordination:

Work with police departments, social service staff, and health care providers to support protective service cases

Data Management:

Enter and maintain consumer information in the SAMS system, including assessments and service histories

Community Outreach:

Provide educational seminars to care providers when requested

Work Schedule and Additional Information:

Full-time employment Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch.

Telework:

You will not have the option to telework in this position.

Salary:

In some cases, the starting salary may be non-negotiable. On call pay is $275 per week + an additional $50 if the week has a holiday in it. You will receive further communication regarding this position via email. Check your email, including spam/junk folders, for these notices.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY QUALIFICATIONS

Minimum Experience and Training Requirements:

Six months as an Aging Care Manager 1 or a County Caseworker 1; or Successful completion of the County Social Casework Intern program; or Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training.
