B/S/S/T Area Agency on Aging has a vacancy for an Aging Care Manager 2 in our Towanda, PA office. This is a full time position (40 hrs. a week; Monday-Friday 8am-4:30pm) with benefits including health insurance, vision and dental insurance along with voluntary supplemental insurance. This full time position comes with paid time off (vacation, sick, holiday and personal) and 401(k) with an Agency contribution after a year of employment. This position is looking for someone who has a passion for working with older adults, veterans and the disabled population in Bradford County, PA. If you are looking to make a difference in someone's life and want to be able to advocate for others, this is the position for you. Responsibilities include maintaining a caseload of older consumers, taking referrals, conducting home visits, completing assessments, developing care plans, arranging for services in the home environment, monitoring ongoing needs of the older individuals, serving as a link to community services, maintaining cooperative working relationships with service providers and ensuring services are being provided as directed. Applicant must be a strong consumer advocate who can maintain a balance of objectivity and empathy. Applicant should have excellent communication skills along with having great computer skills for documentation. This position is housed out of the Towanda, PA office but travel is required. There is mileage reimbursement. Applicant must have a reliable vehicle for work related travel (when a company vehicle is not available), up-to-date vehicle insurance and up-to-date car registration and possess a valid driver's license.

Minimum Experience and Training Requirements:

Six (6) months as an Aging Care Manager 1 or a County Caseworker 1; Or Successful completion of the County Social Casework Intern Program; Or Six (6) months of professional human services or nursing experience; and a Bachelor' Degree in social sciences, behavioral sciences, human services, or a closely related field; Or Any equivalent combination of experience and training.
