Responsibilities Assume leadership for nursing staff during designated shifts, providing guidance, mentorship, and support to ensure optimal patient care. Oversee daily clinical operations, including patient assessments, medication administration, IV infusions, and wound care. Coordinate patient flow and prioritize urgent needs through triage assessments, ensuring timely interventions for acute pain management and emergency situations. Maintain accurate medical documentation using Electronic Health Record (EHR) systems such as Epic or Cerner, adhering to HIPAA and confidentiality policies. Collaborate with interdisciplinary teams to develop and implement individualized care plans for residents with complex needs such as dementia, Alzheimer's disease, or developmental disabilities. Supervise clinical procedures including catheterization, specimen collection, sterile processing, and basic life support techniques like CPR and first aid. Ensure compliance with long-term care regulations and facility policies while fostering a safe environment for patients and staff alike. Experience Valid LPN license with current state licensure required. Proven experience working in long-term care facilities or skilled nursing settings preferred; experience in assisted living or memory care is highly valued. Demonstrated leadership skills in a clinical setting with the ability to manage staff schedules and coordinate patient care effectively. Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), and documentation standards within EMR/EHR systems such as Epic or Athenahealth. Experience in acute care environments including hospital medicine, ICU, emergency room, or trauma centers—particularly Level I or Level II trauma center experience—is advantageous. Familiarity with specialized areas such as hospice care, infectious disease management, ventilator use, tube feeding protocols, and infection control practices. Excellent communication skills with the ability to work collaboratively with multidisciplinary teams while providing compassionate client-centered care to diverse populations—including geriatric patients and individuals with disabilities.
