The RN Clinical Coordinator serves as a central clinical resource for the Behavioral Health program, providing direct patient care, care coordination, and program support. This position collaborates with providers, therapists, case managers, primary care providers, and community partners to ensure coordinated, patient-centered care. The RN Clinical Coordinator supports quality improvement initiatives, monitors high-risk patients, facilitates transitions of care, and promotes adherence to evidence-based behavioral health practices while providing clinical leadership and support to the interdisciplinary team. This position is a Full Time Monday-Friday 8:00am-5:00pm Position.

Essential Duties & Functions:

Performs comprehensive nursing assessments, behavioral health screenings, withdrawal evaluations, crisis assessments, and ongoing monitoring of treatment response in accordance with provider orders and established clinical protocols. Provides direct patient care, including medication education, administration of prescribed medications and long-acting injectable treatments, telephone triage, and patient follow-up. Coordinates patient appointments, referrals, transitions of care, and communication among primary care providers, therapists, case managers, hospitals, emergency departments, and community resources to support continuity of care. Maintains patient registries and tracking systems, monitors high-risk patients and missed appointments, conducts follow-up activities, and supports patient outreach initiatives. Supports departmental quality initiatives through monitoring quality measures, assisting with prior authorizations, and participating in Medication Assisted Treatment (MAT) and other behavioral health programs. Serves as a clinical resource for providers and staff by providing education, supporting workflow development, assisting with protocol updates, and promoting evidence-based practice and continuous quality improvement. Collaborates with the interdisciplinary care team to improve patient outcomes, patient engagement, and operational efficiency. Performs other duties as assigned.

Education:

Graduate of an accredited school of nursing required. Current Registered Nurse (RN) licensure in the State of Kansas required.

Experience:

Minimum of one (1) year of RN experience required. Clinic or ambulatory care experience required. Behavioral health, psychiatric nursing, substance use treatment, or care coordination experience preferred.

Experience with Electronic Medical Records (EMR) systems preferred

Certifications & Licensure:

Current Kansas Registered Nurse (RN) License required Valid driver's license required. Basic Life Support (BLS) certification required. Must obtain within 90 days of hire if not current at the time of hire.
