Job Overview The RAI Coordinator is responsible for coordinating and ensuring the accuracy of the Resident Assessment Instrument (RAI) and Minimum Data Set (MDS) processes. This role ensures compliance with state and federal regulations, supports optimal reimbursement, leads MDS scheduling, and collaborates with interdisciplinary team members to develop comprehensive care plans. The RAI Coordinator reports to the Director of RAI and plays a critical role in quality improvement and accurate clinical documentation. Nursing schedule includes; 40 hours every two weeks Day shift No weekends Lake City Care Center is nestled near the picturesque Lake Pepin in southeast Minnesota. Our skilled nursing facility can provide care for up to 90 residents across our Transitional Care and Long-term care units, supported by a dedicated team committed to Ebenezer's mission of fostering longer, healthier, and more meaningful lives through healing, discovery, and education. Join our experienced healthcare professionals and help us expand our TCU unit. Lake City Care Center Careers (ebenezercares.org) Responsibilities Coordinate and complete MDS assessments and care plans accurately and on time. Manage MDS scheduling, transmission, and billing-related documentation to ensure PDPM accuracy. Provide education and mentorship to staff regarding MDS/RAI processes. Support regulatory compliance and participate in quality improvement initiatives. Collaborate with leadership and serve as a professional role model and resource within the facility.
