Job Description
Job Summary We are seeking a dedicated and detail-oriented RN MDS Clinical Coordinator to join our long-term care team. In this vital role, you will lead the comprehensive assessment and documentation process for residents, ensuring accurate and timely completion of the Minimum Data Set (MDS) assessments. Your expertise will support quality patient care, compliance with federal and state regulations, and reimbursement accuracy. You will collaborate closely with interdisciplinary teams to optimize resident outcomes while maintaining adherence to clinical documentation standards and healthcare policies. Duties Completion of MDS assessments in accordance with CMS (Centers for Medicare and Medicaid Services) guidelines and long-term care regulations. Review medical records, clinical documentation, and coding details such as CPT (Current Procedural Terminology), ICD-9, ICD-10, DRG (Diagnosis-Related Group), and other coding systems to ensure accuracy and completeness. Collaborate with nursing staff, physicians, therapy teams, and other healthcare professionals to gather necessary information for accurate MDS data collection. Conduct thorough documentation review to ensure compliance with HIPAA (Health Insurance Portability and Accountability Act), state healthcare regulations, and clinical documentation standards. Utilize EMR/EHR systems such as Cerner, Epic, Athenahealth, or PCC for data entry, tracking assessments, and maintaining comprehensive medical records. Support utilization management by reviewing patient care plans, discharge planning processes, and ensuring appropriate resource utilization aligned with Medicare and long-term care policies. Provide education on clinical documentation improvement initiatives to staff members to enhance accuracy in medical records and coding practices. Requirements Registered Nurse (RN) licensure with current state licensing requirements. Proven experience with MDS coordination in a long-term care or nursing home setting. Strong knowledge of CMS regulations, Medicare guidelines, and long-term care compliance standards. Familiarity with EMR/EHR systems such as Cerner, Epic, Athenahealth or similar platforms; proficiency in Microsoft Office is preferred. Experience with medical coding including
CPT, ICD-9/10
coding systems; understanding of DRG assignment is advantageous. Critical care experience or ICU background is highly desirable; inpatient or hospital experience adds value. Knowledge of health regulation policies including
HIPAA, NCQA
standards, CMS requirements, and state healthcare regulations related to medical documentation and case management. Excellent attention to detail with strong analytical skills for documentation review and data accuracy. Ability to work collaboratively within multidisciplinary teams while managing multiple priorities efficiently.
