Occupational Therapist, OT La Bella of Edwardsville 6277 Center Grove Road Edwardsville, IL 62025 Join Our In-House Rehabilitation Team! La Bella of Edwardsville is seeking compassionate, dedicated, and motivated Occupational Therapists (Full-Time & Per Diem) to join our growing in-house rehabilitation team . We are committed to delivering exceptional rehabilitation services that help our residents regain independence, maximize function, and improve their quality of life. Our therapists work collaboratively with an interdisciplinary team in a supportive subacute rehabilitation and skilled nursing environment where patient-centered care is our top priority. Position Summary The Occupational Therapist is responsible for evaluating residents, developing individualized treatment plans, and providing evidence-based occupational therapy interventions to improve functional performance and promote safe, successful discharge outcomes. Responsibilities Perform comprehensive occupational therapy evaluations and establish individualized treatment plans. Provide skilled occupational therapy services to residents in a subacute rehabilitation and skilled nursing setting. Develop measurable functional goals and modify treatment plans based on resident progress. Assess and recommend adaptive equipment, assistive devices, and environmental modifications. Educate residents, families, and caregivers regarding treatment plans, safety strategies, and home exercise programs. Collaborate with physicians, nursing staff, physical therapists, speech-language pathologists, and other interdisciplinary team members to coordinate resident care. Complete accurate, timely, and compliant documentation in accordance with Medicare, state, and facility guidelines. Participate in care plan meetings and discharge planning. Maintain productivity expectations while providing high-quality, compassionate care. Ensure compliance with all federal, state, and facility policies and regulations. Qualifications Current Illinois Occupational Therapist license in good standing. Graduate of an accredited Occupational Therapy program. CPR certification preferred. Experience in skilled nursing, subacute rehabilitation, inpatient rehabilitation, or long-term care is preferred, but new graduates are welcome to apply. Excellent communication, clinical reasoning, and documentation skills. Strong organizational and time-management abilities. Ability to work independently while contributing to a collaborative interdisciplinary team. Schedule Full-Time Consistent weekday schedule with occasional weekend rotation as needed.
