What You'll Do As a PRN Home Health Occupational Therapist, you will: Evaluate patients in their homes and develop individualized treatment plans Provide one-on-one occupational therapy services that improve patients' ability to perform activities of daily living (ADLs) Help patients regain independence with self-care, mobility, home management, and other daily tasks Recommend adaptive equipment and home modifications to improve safety and independence Educate patients and caregivers on therapeutic exercises, energy conservation techniques, and adaptive strategies Monitor patient progress and adjust treatment plans as appropriate Complete accurate and timely clinical documentation Collaborate with physicians, nurses, and interdisciplinary healthcare teams to provide coordinated patient care

Qualifications Required:

Current Illinois Occupational Therapist License Excellent communication and interpersonal skills Reliable transportation for travel between patient homes Ability to manage an independent schedule Commitment to providing exceptional patient-centered care

Preferred:
