Position Summary We are seeking a PRN Occupational Therapist to provide in-home therapy services to patients throughout Northwest Indiana. This role offers a flexible schedule, making it an excellent fit for clinicians looking for supplemental hours or greater control over their caseload. Responsibilities

• Conduct occupational therapy evaluations and develop individualized treatment plans based on patient needs and physician orders
• Provide skilled OT interventions in patients' homes to improve function, safety, and independence with daily activities
• Document evaluations, progress notes, and treatment plans in compliance with agency and regulatory standards
• Communicate and collaborate with physicians, nursing staff, and other members of the care team
• Educate patients and caregivers on home safety, adaptive equipment, and self-management strategies
• Monitor patient progress and adjust treatment plans as needed Qualifications
• Current, unencumbered Occupational Therapist license in the state of Indiana
• Degree from an accredited Occupational Therapy program; NBCOT certification required
• Home health experience preferred but not required
• Valid driver's license, reliable transportation, and current auto insurance
• Strong communication and time-management skills; ability to work independently What We Offer

Pay:

$60.00 - $75.00 per hour
