Job Description

Renewal Rehab is seeking a Licensed Occupational Therapist to join our collaborative team serving primarily seniors in skilled nursing and post-acute care settings. In this patient-centered role, you will evaluate and treat residents, develop individualized, outcome-focused plans of care, and help patients maximize safety, independence, and quality of life. You will collaborate closely with an interdisciplinary team, document in EMR, and educate patients and families. Responsibilities Evaluate patients' functional status, cognition, and ADL performance in skilled nursing and post-acute settings. Develop and implement individualized, evidence-based occupational therapy plans of care focused on safety and independence. Provide direct treatment including ADL retraining, functional mobility, cognitive and neuromuscular interventions, and adaptive equipment training. Document evaluations, daily notes, progress reports, and discharge summaries accurately and timely in EMR systems. Collaborate with an interdisciplinary team, including PT, SLP, nursing, and physicians, to coordinate patient-centered care. Educate patients, families, and caregivers on home programs, safety strategies, and equipment recommendations. Participate in care conferences and discharge planning to support smooth transitions and optimal outcomes. Ensure compliance with regulatory and facility guidelines, professional standards, and ethical practice. Contribute to a collaborative, positive clinic culture aligned with Renewal Rehab's mission and values. Engage in ongoing professional development, mentorship, and continuing education opportunities.
