The ideal candidate will possess a strong commitment to improving the quality of life for individuals with disabilities and will work in various settings, including home health, outpatient clinics, and inpatient facilities. This role requires a thorough understanding of anatomy and the ability to develop tailored treatment plans that address the unique needs of each patient. Duties Conduct comprehensive assessments of patients to determine their needs and develop individualized treatment plans. Implement therapeutic interventions aimed at enhancing patients' ability to perform daily activities. Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure coordinated care. Provide education and support to patients and their families regarding treatment goals and progress. Maintain accurate documentation of patient progress and treatment outcomes in compliance with regulatory standards. Engage in early intervention strategies for pediatric patients to promote developmental skills. Manage cases effectively, ensuring that all aspects of patient care are addressed in a timely manner. Experience Proven experience working with individuals with disabilities across various settings such as home health, outpatient clinics, or inpatient facilities is highly desirable. Familiarity with pediatric care and early intervention techniques is a plus. Strong case management skills to coordinate care effectively among multiple providers. In-depth knowledge of anatomy and its application in therapeutic practices is essential. Ability to adapt treatment approaches based on individual patient needs and progress.
