Maybe decreased strength is making meal preparation difficult. Maybe a recent hospitalization has changed their ability to dress independently. Maybe they're afraid of falling and have stopped doing activities that once defined their independence. You aren't working from hypotheticals. You can see the challenge in front of you and develop interventions around the patient's actual environment. That's a powerful way to practice Occupational Therapy. What You'll Be Doing Perform comprehensive in-home Occupational Therapy evaluations. Assess ADLs, functional abilities, home safety, and barriers to independence. Develop individualized treatment plans based on patient needs and goals. Provide interventions focused on strength, coordination, motor skills, and functional performance. Address bathing, dressing, grooming, meal preparation, and other activities of daily living. Recommend adaptive equipment and home modifications. Educate patients and caregivers on safety and compensatory strategies. Collaborate with nurses, PTs, physicians, and other interdisciplinary clinicians. Complete accurate and timely clinical documentation. Monitor progress and modify plans of care as appropriate. Home Health Experience Is Valuable, But Great OT Experience Comes From Many Settings If you're currently working in Outpatient Therapy, Inpatient Rehabilitation, Skilled Nursing, Acute Care, Transitional Care, Senior Living, Long-Term Care, or another rehabilitation setting , your experience may translate exceptionally well. We're looking for an Occupational Therapist who is clinically confident, creative, organized, and comfortable practicing independently. Required Qualifications Active Minnesota Occupational Therapist license OTR/L Valid driver's license and reliable transportation Strong clinical reasoning and assessment skills Excellent patient and caregiver communication Ability to independently manage a field-based schedule Preferred Qualifications Home health experience Experience with medically complex or geriatric populations Familiarity with EMR systems Strong interest in functional, patient-centered rehabilitation Practice OT Where Independence Actually Happens You became an Occupational Therapist to help people do the things that make life feel like their own.
