Job Description

Jackson Therapy Partners is seeking an Occupational Therapist to provide patient-centered therapy services across diverse clinical settings. You'll evaluate and treat patients, develop individualized treatment plans, collaborate with interdisciplinary teams, and document progress in accordance with facility and regulatory standards. Responsibilities Perform comprehensive occupational therapy evaluations and re-evaluations using appropriate standardized and functional assessments Develop, implement, and modify individualized treatment plans based on patient goals, functional status, and evidence-based practice Provide skilled interventions focused on ADLs, IADLs, functional mobility, upper extremity function, cognition, and safety Educate patients, families, and caregivers on therapeutic techniques, home exercise programs, and strategies to improve independence and safety Recommend, fit, and train patients in the use of adaptive equipment, assistive devices, and durable medical equipment Document all evaluations, daily treatments, progress notes, and discharge summaries accurately and within required time frames Collaborate with physicians, nurses, rehabilitation professionals, and case managers to coordinate and optimize patient care Participate in interdisciplinary care conferences and contribute to discharge planning and goal setting Adhere to state and federal regulations, payer policies, and professional practice standards for occupational therapy Maintain professional development and stay current with clinical best practices and emerging treatment approaches Required Skills Occupational therapy evaluation and treatment planning Knowledge of anatomy, kinesiology, and rehabilitation principles ADL and IADL retraining Neuromuscular reeducation and therapeutic exercise Cognitive and perceptual rehabilitation techniques Functional mobility and transfer training Splinting and upper extremity rehabilitation (basic to intermediate) Adaptive equipment assessment and training Patient and family education Clinical documentation and EMR proficiency Interdisciplinary team collaboration Time management and caseload organization Knowledge of payer and regulatory requirements (Medicare, commercial insurers) Professional communication and counseling skills
