The ideal candidate will have a strong background in patient care, with experience in various settings including clinics, home health, and inpatient facilities. This role is essential in helping patients regain their independence and improve their quality of life through tailored therapeutic interventions. The Occupational Therapist will work closely with patients of all ages, particularly geriatrics, to assess their needs and develop effective treatment plans. Duties Conduct comprehensive evaluations of patients' physical, emotional, and social needs to determine appropriate therapy goals. Develop individualized treatment plans that address the specific needs of each patient, incorporating techniques from various therapy disciplines including speech therapy when necessary. Implement therapeutic interventions aimed at improving patients' daily living skills and overall functionality. Collaborate with interdisciplinary teams to ensure cohesive care and effective discharge planning for patients transitioning from inpatient to home health settings. Maintain accurate documentation of patient progress and outcomes in compliance with regulatory standards and organizational policies. Provide education and support to patients and their families regarding treatment options, home exercises, and adaptive equipment usage. Engage in case management activities to coordinate care among various healthcare providers involved in the patient's recovery process. Requirements A valid Occupational Therapy license in the state of practice. Bachelor's or Master's degree in Occupational Therapy from an accredited program. Strong knowledge of human physiology and anatomy as it relates to occupational therapy practices. Experience in geriatrics or working with elderly populations is preferred. Familiarity with discharge planning processes and case management principles. Excellent communication skills for effective interaction with patients, families, and healthcare professionals. Ability to work independently as well as part of a collaborative team environment. Previous experience in clinic settings or home health care is a plus but not mandatory.
