SUMMARY:

Provides case management and direct patient care within a team environment. Administers comprehensive skilled care.

Schedule:

Monday - Friday, 8 to 430pm No weekends, no holidays, No on calls

Location:

Seneca County Qualifications Education & Experience - Applicants must possess the following qualifications or an equivalent combination of education and experience:

Education:

Graduate of an approved Occupational Therapy program.

Experience:

Minimum of one-year experience in a hospital inpatient setting.

Licensure:

Possession of current OT license to practice in New York State. Responsibilities Provide skilled OT services to patients in their homes. Complete patient evaluations and develop individualized treatment and discharge plans. Deliver interventions to improve independence, safety, self-care, and daily living skills. Educate patients, families, and caregivers on treatment plans, adaptive strategies, and home safety. Monitor patient progress and modify treatment plans as needed. Collaborate with an interdisciplinary care team to coordinate patient-centered care. Supervise COTAs as applicable. Maintain timely, accurate clinical documentation. Communicate changes in patient condition and coordinate appropriate follow-up. Connect patients with community resources and support services. Participate in care conferences, education, quality improvement, and team initiatives. Maintain required productivity, quality, and regulatory standards.

Salary:

$67,700 - $115,045.00 #URM1
